The Annual Assembly of Hospice and Palliative Care 2017 – ACCA presents on the Advanced Illness Care Program

The 2017 Annual Assembly of Hospice and Palliative Care join brought together more than 3,000 experts in hospice and palliative care across the US to Phoenix, Arizona, February 22-25. ACCA’s Executive Director, Cynthia Carter Perrilliat, and Program Manager, Valerie Steinmetz, presented the ACCA Advanced Illness Care Program, “Bridging the Gap Between the Health Delivery Systems, Community Organizations, and Faith-Based Communities in Managing Advanced Illness.” Through discussion, use of case examples, and review of ACCA Advanced Illness Care Program pilot data, the presentation provided practical information for forming partnerships with faith-based community led organizations and how to reach vulnerable populations with advanced illness care needs beyond the healthcare delivery system through the training and deployment of community care navigators.
