American Public Health Association 2017 Conference – ACCA Poster Presentation

The American Public Health Association’s 2017 annual meeting brings together experts from all fields of public health, clinical, and policy and community to improve the health of all people and all communities.  The Alameda County Care Alliance (ACCA) participated in a themed poster presentation session – Improving the Nation’s Health through Faith-based Approaches on November 7, 2017 in Atlanta, Georgia. Pastor Reems from Center of Hope and Dr. Janice Bell from the UC Davis Betty Irene Moore School of Nursing presented a poster on ACCA’s Advanced Illness Care Program: A Novel Faith-Based Approach to Addressing Advanced Illness Care Disparities.  The poster provides an overview of the ACCA history and formation, the Advanced Illness Care Program big 5 cornerstones and subset of results, the ACCA workforce, important program partnerships and highlights key dissemination activities.
